Atrophic, rolling, and icepick scars are anchored by deep dermal fibrotic bands. Here is the clinical rationale behind subcision, TCA CROSS, and radiofrequency microneedling.
The Anatomy of Pitted Acne Scars
When cystic acne heals with severe collagen depletion, fibrotic bands form beneath the dermis. These fibrous tethers pull the epidermis down, creating depressions known as rolling, boxcar, or icepick scars. Because these scars are structural, topical serums and cosmetic creams cannot penetrate deeply enough to release the tethered tissue.
How Nokor Needle Subcision Releases Fibrotic Bands
Subcision is a minor dermatosurgical procedure performed under local anaesthesia. A specialized Nokor needle or micro-cannula is inserted into the subdermal plane to sever the fibrotic strands anchoring the scar. Releasing these bands elevates the depression, while the controlled micro-injury triggers a healing cascade that stimulates autologous collagen synthesis beneath the scar.
TCA CROSS for Deep Icepick Scars
Icepick scars are narrow, deep columns that extend into the reticular dermis where subcision cannot reach. Chemical Reconstruction of Skin Scars (TCA CROSS) involves the focal application of high-concentration (70%–100%) trichloroacetic acid directly inside the scar pit, coagulating epithelial tissue and stimulating progressive bottom-up collagen filling.
Collagen Induction via Fractional RF Microneedling
To smooth superficial texture and remodel the surrounding dermal matrix, fractional radiofrequency (RF) microneedling delivers thermal coagulation at precise depths (0.5mm to 3.5mm) without overheating the melanin-rich epidermis, making it exceptionally safe for Indian Fitzpatrick skin types III to V.
Expected Clinical Downtime & Session Planning
Downtime is typically 3 to 5 days of mild swelling and superficial crusting. Because neo-collagenesis takes 6 to 12 weeks, clinical sessions are spaced 4 to 6 weeks apart over a course of 3 to 5 sittings for cumulative, permanent textural restoration.
